Harborside Psychiatrics is based on Cape Cod, Massachusetts, and delivers medication management by telehealth to clients across New England, including Connecticut. Darien households are used to choosing a clinician on purpose, the same way they choose a surgeon, an attorney, or a school: on fit and on the specifics of how care is delivered, not on who happens to be nearby. This page is written for that kind of decision.
If you are weighing whether a video-based prescriber can actually manage a medication with the same attention as an in-person visit, the sections below explain how it works, what it costs, and who is doing the prescribing.
Harborside is based in Yarmouth Port, Massachusetts, and every appointment is delivered by secure video. You do not need special equipment. A laptop, tablet, or smartphone with a camera and a private room to sit in is enough. Appointments are scheduled at a set time, the same as an in-person visit, and you log in through a secure link a few minutes before the session starts.
An initial medication evaluation runs longer than a follow-up, because your prescriber is building a full picture of your history, current symptoms, and any medications you have already tried. Follow-up appointments are shorter and are spaced according to what you are being treated for and how your prescriber wants to monitor it, sometimes closer together at the start and further apart once things are stable.
If a medication is prescribed, it is sent electronically to a pharmacy of your choosing, so you can pick it up locally in Darien or wherever is most convenient for you. Your prescriber can also send refills and adjust dosing between appointments as needed, using the same secure system.
Medication management at Harborside addresses conditions like anxiety, depression, panic, obsessive-compulsive symptoms, attention and focus difficulties, mood instability, and sleep disruption tied to a mental health condition. Functional psychiatry visits also look at how nutrition, sleep, hormones, and general physical health interact with mental health symptoms, since those pieces often need attention alongside a medication plan.
Every plan starts from a conversation about what you are experiencing day to day, not a diagnosis alone. Some people come in already on a medication and want a second opinion or a change. Others have never taken anything and want to understand what their options actually are before deciding.
The first visit is a full evaluation. Your prescriber will ask about your current symptoms, your medical and psychiatric history, any medications or supplements you take now, and what has or has not worked in the past. Expect to talk about sleep, appetite, energy, family history, and how symptoms are affecting your work and relationships in Darien and beyond.
By the end of that appointment, you and your prescriber will have a plan: that might mean starting a medication, adjusting one you already take, ordering labs, or deciding to hold off and reassess at a follow-up. You will leave with a clear next step, not just a diagnosis.
Medication management and functional psychiatry at Harborside are performed by psychiatric nurse practitioners: Rachel Egan, RN PMHNP-BC, who is also the clinical director, Emily Pardue, RN PMHNP-BC, and Kirstin Ritchie, RN PMHNP-BC. Each is qualified to evaluate, diagnose, and prescribe for mental health conditions. Full bios and credentials for each clinician are on the team page.
Therapy and eating disorder care, when part of a client’s plan, are delivered by Harborside’s licensed therapy team: Hannah Legesse, LICSW, Tori Mahoney, LMHC, Jacquelyn Duford, LCMHC ATR, and Lexie Carchedi, LCSW. Medication management and therapy can run alongside each other or on their own, depending on what you need.
Harborside is a private-pay practice and does not bill insurance directly for appointments. Current fees for an initial and a follow-up medication management appointment are listed on the pricing page. If you have questions about cost, you can ask before anything is booked.
You can request an appointment online or reach out with questions first. Once you are scheduled, you will receive a link to complete some intake information ahead of your first visit, so more of that appointment time can go toward talking with your prescriber. From there, Darien clients are seen on the same video platform for every appointment, initial and follow-up alike, with prescriptions sent to the pharmacy you choose.
To learn more about the service generally, visit the medication management hub, or see how Harborside works across the state on the Connecticut telehealth page and the Lower Fairfield County region page. If anxiety is part of what brought you here, the anxiety treatment page goes into more detail on that specific condition.

No. The first appointment includes a full evaluation, so a prior diagnosis isn’t required to get started.
Yes, with your permission. Many patients keep an existing therapist and use Harborside only for medication management.
It depends on the medication and how you’re responding to it. Some patients are seen monthly at first, then less often once things are stable.
Prescriptions are sent electronically to the pharmacy of your choice, the same as they would be after an in-person visit.
Harborside is private-pay and doesn’t bill insurance directly. A superbill is available for out-of-network reimbursement, where a plan allows it.
That’s part of ongoing management. Changes are discussed and made together at a follow-up visit, not on your own.
