Harborside Psychiatrics is based on Cape Cod, in Yarmouth Port, Massachusetts, and delivers care by telehealth to clients across New England, including Wilton. Wilton households are used to choosing a specialist on purpose, not settling for whoever is available, and many families here already pay out of network for physicians, tutors, and specialists they’ve vetted themselves. This page is written for that kind of decision: what medication management with our team actually involves, who provides it, and how the video-based model works day to day.
If you’re looking into medication management, you’re probably weighing a few things at once: whether a prescriber will actually listen, how quickly you can get on the calendar, and whether the process will feel rushed or thorough. Those are reasonable things to want answered before you book anything.
Our approach is straightforward. A psychiatric nurse practitioner evaluates your history, current symptoms, and any medications you’ve already tried, and works with you to build a plan. That plan might include medication, might not, and is always something you understand and agree to before it starts. For people in Wilton balancing demanding jobs, commuter schedules, and family responsibilities, the appeal of this model is usually less about convenience and more about getting a clinician who has time to actually think about your case.
All appointments happen over secure video, using a link sent ahead of time. You’ll need a device with a camera and a private space to talk, whether that’s a home office in Wilton or a quiet room during a workday. No special software or app download is required beyond what’s provided in the appointment confirmation.
Initial evaluations typically run longer than follow-ups, since there’s more history to cover. Follow-up medication management appointments are shorter and are scheduled at an interval your prescriber sets based on your situation, often more frequent early on and less frequent once things are stable. If a medication is prescribed, it’s sent electronically to the pharmacy of your choice, so you can pick one near your home or office in Wilton, or anywhere else that’s convenient.
Harborside is based in Yarmouth Port, Massachusetts. Your clinician works from there or from another Harborside location, and you connect from wherever you are in Connecticut. The clinical relationship, the history-taking, the follow-through between visits, works the same way it would in person. What changes is that you’re not driving anywhere to get it.
Medication management at Harborside covers a broad range of conditions affecting mood, thinking, and daily functioning. This includes depression, anxiety, panic disorder, bipolar disorder, obsessive-compulsive disorder, ADHD, insomnia, and other conditions that respond to medication as part of a treatment plan. It also includes functional psychiatry, which looks at how nutrition, sleep, hormones, and physical health interact with mental health symptoms, for people who want that piece considered alongside medication.
Every plan is individual. Some people need an evaluation and a single medication reviewed periodically. Others benefit from medication alongside therapy, and our team can coordinate that when it’s part of your care.
The first appointment is an evaluation, not a prescription handed over on the spot. Expect to talk through your current symptoms, your health and medication history, anything you’ve already tried, and what you’re hoping care will change. Your clinician will ask direct questions and give you space to ask your own.
By the end of that appointment, you and your clinician should have a shared understanding of what’s being addressed and what the plan looks like, whether that includes medication right away, a period of monitoring, or a referral for therapy alongside it.
Medication management and functional psychiatry at Harborside are performed by psychiatric nurse practitioners: Rachel Egan, RN PMHNP-BC, who is also Harborside’s clinical director, Emily Pardue, RN PMHNP-BC, and Kirstin Ritchie, RN PMHNP-BC. Each is qualified to evaluate, diagnose, and prescribe for mental health conditions. Full bios and credentials for each of them are available on our team page.
Therapy, including eating disorder care, is delivered by our licensed therapy team: Hannah Legesse, LICSW, Tori Mahoney, LMHC, Jacquelyn Duford, LCMHC, ATR, and Lexie Carchedi, LCSW. Depending on your needs, your care may involve one of our prescribers, one of our therapists, or both working together.
Harborside is a private-pay practice and does not bill insurance directly for appointments. Current fees for an initial and a follow-up appointment are listed on our pricing page, and questions about cost can be answered before anything is booked.

Yes, with your permission. Many patients keep an existing therapist and use Harborside only for medication management.
It depends on the medication and how you’re responding to it. Some patients are seen monthly at first, then less often once things are stable.
Prescriptions are sent electronically to the pharmacy of your choice, the same as they would be after an in-person visit.
Harborside is private-pay and doesn’t bill insurance directly. A superbill is available for out-of-network reimbursement, where a plan allows it.
That’s part of ongoing management. Changes are discussed and made together at a follow-up visit, not on your own.
