Functional Psychiatry for Stow, MA

A traditional psychiatric work-up usually starts and ends with symptoms: what you feel, how often, how severe. Functional psychiatry starts earlier than that. It looks at sleep, nutrition, hormones, inflammation, and other physiological factors that can drive mood, focus, and anxiety, and it uses targeted testing to check for deficiencies or imbalances that a symptom checklist alone would miss. In a town like Stow, where families tend to juggle two working parents, long commutes to the 495 corridor or into Boston, and kids moving between school and multiple activities, it is often a parent doing the research at 10pm trying to understand why a teenager’s energy or mood has changed, or why their own has. That kind of question deserves more than a quick medication swap.

Functional Psychiatry for people in Stow, MA

Harborside Psychiatrics is based in Yarmouth Port, Massachusetts, and delivers care by video to patients across the state, including Stow. You do not travel to an office. You need a private space, a reasonably stable internet connection, and a phone, tablet, or computer with a camera. That is the whole technical requirement.

Appointments are scheduled at set times, the same as an in-person visit would be. An initial evaluation runs longer, generally around an hour, so there is time to go through history, current concerns, and whether lab-based testing makes sense for your situation. Follow-up appointments are shorter and are spaced based on what is being managed and how a treatment plan is progressing. If medication is prescribed, it is sent electronically to a pharmacy of your choosing near Stow, the same as any other prescription.

If testing is part of your plan, your clinician will explain what is being ordered and why, and how the results factor into decisions about your care. Nothing is ordered as a routine add-on. It is tied to a specific clinical question.

How telehealth care actually works

Functional psychiatry at Harborside is often a fit for people who have already tried a standard approach to anxiety, depression, attention and focus concerns, or persistent fatigue and mood changes, and want a clinician who will also look at contributing physical factors: sleep quality, thyroid and hormonal function, nutrient status, and other markers that can affect how you think and feel. It is also used for people who are earlier in figuring out what is going on and want an evaluation that does not stop at a diagnosis code before considering what might be underlying it.

This is not a replacement for emergency or crisis care, and it is not a general wellness or longevity service. It is a psychiatric approach that takes physiological information into account alongside a standard clinical evaluation.

What we treat

The first appointment is a conversation before it is anything else. Your clinician will ask about your current symptoms, your medical and psychiatric history, medications or supplements you take, sleep, diet, and family history. From there, they will talk with you about whether targeted lab testing is appropriate, what it would look for, and what a reasonable next step looks like. You will leave the appointment with a plan, not just a prescription. If medication is part of that plan, you and your clinician will discuss options, expected considerations, and how you will be monitored going forward.

What the first appointment involves

Medication management and functional psychiatry at Harborside are performed by psychiatric nurse practitioners: Rachel Egan, RN PMHNP-BC, who is also Harborside’s clinical director, Emily Pardue, RN PMHNP-BC, and Kirstin Ritchie, RN PMHNP-BC. Each is qualified to evaluate, diagnose, and prescribe for mental health conditions, and each brings their own areas of clinical focus to that work. Full bios and credentials for all three are on the team page.

When therapy is part of a plan, including support for eating disorders, that work is carried out by Harborside’s licensed therapy team: Hannah Legesse, LICSW, Tori Mahoney, LMHC, Jacquelyn Duford, LCMHC ATR, and Lexie Carchedi, LCSW. Many patients in Stow work with a prescriber and a therapist together, coordinating on the same treatment plan.

Who our clinicians are

Harborside is a private-pay practice and does not bill insurance directly for appointments. Current fees for an initial and a follow-up appointment are listed on the pricing page, and questions about cost can be answered before anything is booked.

Frequently Asked Questions

Is functional psychiatry the same as functional medicine?

No. It’s psychiatric care from a licensed psychiatric nurse practitioner that also reviews lab work and lifestyle factors, not a separate functional medicine practice.

No. Whether labs make sense is decided together at the first visit, based on your history and symptoms.

Bring it to your first appointment. Existing results can often be used instead of repeating tests.

No. Labs are typically billed separately by the lab itself and may be partly covered by insurance depending on the plan.

Yes. It’s meant to work alongside an existing therapist or primary care provider, not replace them.

Functional psychiatry includes the same prescribing and monitoring, plus a review of physical health factors like sleep, nutrition, and lab markers.
